HIPAA Notice of Privacy Practices

Belaray Dermatology PC is required by law to maintain the privacy of your protected health information (PHI) and to provide you with notice of our legal duties and privacy practices. We are committed to protecting medical information about you.

We may use and disclose your health information for treatment, payment, and healthcare operations without obtaining your separate written authorization, as permitted under the Health Insurance Portability and Accountability Act (HIPAA). Examples include:

  • Treatment: Sharing information with other providers involved in your care
  • Payment: Submitting claims to your insurance company
  • Operations: Quality assurance, compliance, and administrative functions

For uses beyond treatment, payment, and operations, we will request your written authorization. You may revoke any authorization in writing at any time.

Website Privacy Policy

This website collects personally identifiable information only when you provide it, for example by submitting a form or by texting or calling us. Clicking a scheduling, bill-pay, or patient-portal link takes you to a separate service with its own privacy policy. We do not sell personal information, and we do not share it with third parties for cross-context behavioral advertising.

Like most websites, our web server and the analytics tools described below receive technical information from your browser when you visit, such as your IP address, browser type, the pages you view, and the site that referred you. We use this information only to understand aggregate traffic and to keep the site working. It is never combined with your medical record.

Cookies, analytics, and third-party services

This site runs no advertising, remarketing, or social-media tracking pixels. The following tools are in use:

  • Google Analytics 4 (Google LLC): sets two first-party cookies (_ga and _ga_*, kept for up to two years) to count visits and page views. We have disabled Google Signals and ad personalization, so Google does not use this data to build advertising profiles. Google's own policies govern its processing: How Google uses information from sites that use its services.
  • Plausible Analytics (Plausible Insights OÜ, hosted in the EU): cookieless, privacy-focused page-view counting. It stores no cookies and no personal identifiers.
  • Google Reviews widget (Elfsight) on the homepage: displays our public Google reviews and sets two cookies of its own (one session cookie and one that expires within a day).
  • Weather and map data on the homepage and the UV index page come from Open-Meteo and OpenStreetMap. These requests carry no personal information beyond your IP address.
  • Fonts are loaded from Google Fonts.

Pages where you enter personal or payment information, including online bill pay, carry no analytics at all.

Your privacy choices

  • Global Privacy Control. We honor the Global Privacy Control (GPC) browser signal. When your browser sends it, Google Analytics is not loaded on any page of this site. Learn how to turn it on at globalprivacycontrol.org.
  • Browser settings. You can block or delete cookies in your browser settings. The site works without them.
  • Google Analytics opt-out. Google offers a browser add-on that stops Google Analytics on every site.
  • Questions or requests. To ask what information we hold about you, or to request its deletion, contact us using the details below.

Mobile numbers and text messaging

Mobile phone numbers collected for text messaging are used only to send the messages described in our SMS terms. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Sharing with service providers that help us deliver messages (for example, our messaging platform) is limited to that purpose. Text messaging opt-in data and consent are never sold or shared with any third party.

Links to third-party services (patient portal, online scheduling, bill payment) are subject to those services' own privacy policies.

SMS text messaging terms

Belaray Dermatology PC sends text messages (SMS) to people who have agreed to receive them. Patients may receive appointment reminders, visit instructions, follow-up messages, and replies to texts they send us. Staff members may receive scheduling and timekeeping reminders, such as a reminder to clock in. Patient texting is described in more detail on our electronic communications page.

  • Message frequency varies by person and purpose.
  • Message and data rates may apply.
  • Reply STOP to any message to stop receiving texts from that number. Reply HELP for help, or call (516) 822-7546.
  • Consent to receive text messages is not a condition of receiving care or of employment.
  • Wireless carriers are not liable for delayed or undelivered messages.
